Welcome to ALT's clearance page!
Below are specific instructions on how to fill out the clearance packet online. Our objective is to make this process as easy and expedient as possible for everyone involved in the process, especially you! It might also help to print this page so you may have something to refer to.
- The first thing you want to do is download the form by clicking on the link below that says "Standard Form 86". You will need the FREE Adobe Acrobat Reader 6 software. A link is provided to the right of the screen to download it if you need it.
- Follow the directions and enter in the brief registration information.
- If there is a question that doesn't apply to you, simply fill in the answer as "N/A" (Not Applicable) They (DEA) want you to acknowledge that this section does not apply to you, and the section was not skipped over. If there is an entire section that does not apply to you, it is not necessary to fill in every space.
- Make sure all of the information is true and correct. We cannot stress enough how important it is to be completely honest.
- If there is a question you do not understand, refer back to the very beginning part of the form where it outlines the directions. If you still have questions after you have done this, give us a call and we will be more than happy to assist you.
- Make sure you fill out your Social Security Number on EVERY page in the space provided at the bottom
- This process requires that all forms requiring signatures and dates be ALL original on all three forms. The best way to approach this would be to:
- Fill out all necessary information on every page except where it asks for a signature or date.
- Save a copy of the file to your hard disk, and e-mail the form to juanita@Americanlt.com so that we may make any necessary corrections.
- After all of the corrections have been made and the final packet has been approved, make two additional copies of the of the SF-86 to total 3 copies.
- Once you have made copies, go back and input original signatures and dates on ALL three copies. Incomplete paperwork will be sent back for correction.
- You will also need to have three (3) sets of fingerprints taken from your local law enforcement agency. If they cannot do this for you they can point you in the direction of where these services are available. ALT will provide you the fingerprint cards via mail.
